The window washer who fell dead while working on a Coral Gables building has been identified as the investigation into the incident continues.
Elmer Solís, 43, is the man who died Wednesday, according to the Miami-Dade coroner.
Solís was working on a scaffolding in the Giralda Place condominium building near Salzedo Street and the Alhambra Circle when it fell around 2 p.m.
Miami-Dade police, who are investigating the incident, said Thursday that there appears to have been a failure in the equipment while the man was working.
NBC’s Victor Jorges 6 has more information about what caused the man to lose his life Wednesday.
“It’s actually been a real shock to the window cleaning community,” said Stefan Bright, director of security for the International Window Cleaning Association. “We haven’t seen so many fatalities in the last decade.”
Bright said it may have been too early to determine what part of the man’s team failed before he fell to his death, but everything could have been avoided.
“We provide safety training and education to our members who are primarily window cleaning contractors in the United States,” he said.
The Occupational Safety and Health Administration told NBC 6 that the company that hired the man is called QBS Cleaning Corporation, which operates as Quality Building Services.
NBC 6 contacted QSB for its security requirements and protocol. They have not responded.
Bright said the company is not part of IWCA and the association has no record of anyone from that company participating in or attending training sessions.
Subscription is optional, but Bright said the services and training they offer can help prevent tragedies.
“It almost seems like this is a case where there could have been a lapse in training or something was lost,” he said. “I don’t like to talk without hearing the result of the research, but it’s seven or eight steps you have to follow and you learn it in your training. It’s obvious that something was lost here.”
The Miami-Dade forensic doctor said Solis died of a severe trauma.
